About Hawaii Health Systems Corporation
HHSC was established by the Hawaii legislature through Act 262 in 1996, transitioning community hospitals from county control to a public benefit corporation model aimed at increasing autonomy and reducing bureaucratic constraints.
HHSC is an instrumentality of the State and is administratively attached to the Department of Health; it is governed by a corporate board appointed by the governor, and individual regional boards oversee operations across five geographic regions. The corporate board typically includes the regional CEOs and others, aligning governance across the state.
HHSC operates multiple facilities: in East Hawaii (Hilo Benioff Medical Center, Honokaʻa Hospital—Hale Hoʻola Hamakua, Ka‘ū Hospital), West Hawaii (Kona Community Hospital, Kohala Hospital), Kauai Region (Kauai Veterans Memorial Hospital, Samuel Mahelona Memorial Hospital), and Oahu (Leahi Hospital, Maluhia, Kahuku Medical Center). Some former facilities in the Maui region were transferred to Kaiser Permanente in 2017.
HHSC functions as a safety-net provider, offering both acute hospital care and long-term skilled nursing services. It operates licensed beds across multiple islands, serving a broad patient base irrespective of payment ability; as of FY 2018, it operated 847 licensed beds and delivered tens of thousands of patient days and admissions. The system employs approximately 2,500 staff, with significant unionization, and is supported by state legislative appropriations.
