Creative Solutions operates exclusively in Texas. Its footprint has expanded through a series of acquisitions—including five facilities from Diversicare in July 2025, another five in September 2025, and two more in November 2025, bringing the count to 182 long‑term care communities.

Beyond skilled nursing, CSNHC has also pursued real estate acquisitions, including the purchase of Wes‑Tex Printing in November 2025—a separate, family‑related business. Additionally, the chain has leveraged HUD‑insured financing to acquire 18 nursing facilities representing over 2,200 licensed beds in the 18 months preceding August 2025.
