About Quality Life Services
Quality Life Services (QLS) is a family-owned provider of senior care in Western Pennsylvania, operating a collection of skilled nursing facilities and associated services to support communities in the region.
The company originated in 1956, when Ken and Nancy Tack purchased a farm that later became the flagship nursing home, Quality Life Services – Sugar Creek. The skilled nursing addition was built four years later. In the early 1990s, the second generation (Ken and Phil Tack) took over operations. The third generation—Steven Tack, Susie Tack‑Beardsley, and Mary Susan Tack‑Yurek—assumed ownership and leadership in 2007.
QLS remains family-owned and operated, with operations run by third-generation family members. It appears to be privately held; there is no indication of public ownership or PE backing.
QLS operates approximately 10 skilled nursing facilities all located in Pennsylvania, specifically in Western PA (e.g., Apollo, Chicora, Grove City, Henry Clay, Markleysburg, Mercer, New Castle, Sarver, Sugar Creek, Westmont).
QLS provides a range of services, including: Skilled Nursing & Rehabilitation, In-Home Healthcare, Hospice Care, Personal Care / Assisted Living. A non-profit initiative 'Don’t Stop Dreamin’' aimed at fulfilling once-in-a-lifetime dreams for residents and patients. Additional partnerships include Quality Pharmacy.
