About Monument Health Group

Monument Health Group is a growing provider of skilled nursing and rehabilitation services in Utah. It entered the Utah healthcare market in February 2024 by acquiring its first two facilities in Millcreek and Bountiful, with leadership experienced from operations in Arizona. In early 2025, the group expanded by acquiring nine additional facilities, establishing a statewide network.

The network operates 11 skilled nursing facilities across Utah, with locations including Millcreek, Bountiful, Canyon Rim, Brigham City, South Salt Lake, Pioneer Trail, American Fork, Murray Creek, Stonecreek, Taylorsville, and Cottonwood Creek.
