About Mission Point Healthcare Services
Mission Point Healthcare Services (MPHS) comprised a for-profit chain of skilled-nursing and rehabilitation centers under a parent operating entity, serving families in Michigan and Nevada. Their facilities operated as individual limited liability companies under a centralized management and reimbursement framework.
MPHS used a layered structure wherein each facility was a separate LLC, while operations were conducted through Mission Point Healthcare Services LLC, with managerial control by Mission Point Management Services LLC. Individual facility ownership often involved separate holding LLCs.
In late 2025, Mission Point’s operating businesses were sold to an investment group associated with another chain (Villa) for a nominal amount, while liabilities were assumed. Legal action had already been initiated on eight facilities due to rent defaults, loan defaults, and unpaid taxes.
As of early 2026, CMS-based data confirms MPHS operated 21 affiliated facilities spanning Michigan and Nevada.
Their primary line of business was operating post-acute care, skilled nursing, and rehabilitation services. The chain’s organizational design allowed purchase or divestiture of individual facilities with minimal interdependence.
