Health Services Management, Inc. was formed in February 2000 when it leased six skilled‑nursing facilities in Florida that had been in bankruptcy and needed operational and financial turnaround. Since then, it has grown into a multi‑state nonprofit operator, managing skilled‑nursing and assisted‑living communities with capacity to serve over 2,300 residents.

It specializes in a range of services, such as short‑ and long‑term rehabilitation, post‑acute skilled nursing, outpatient care, memory care, respite, palliative care, and assisted living. HSM operates in Florida, Tennessee, and Texas, with its corporate headquarters located in Crystal River, Florida.
