Health Dimensions Group (HDG) was established in 2000 with the mission of improving older adults’ lives by delivering care, service, and consulting to health systems and senior living providers nationwide.
The company is privately held and women‑owned, serving both nonprofit and for‑profit organizations including health systems, PACE organizations, REITs, faith-based, and government entities.
Over its history, HDG has expanded into two major lines of business: direct management of senior living and care communities in nine states, and broad consulting services—including strategy, start‑up support, development, staffing, marketing, and turnaround assistance—across all 50 states.
