About Evergreen Healthcare Group

Evergreen Healthcare Group began in April 2020 in Washington, initially managing two skilled nursing communities. It has since grown through a series of acquisitions—in California (2021, 2023), Nevada (2021), and notably the EmpRes Healthcare portfolio in September 2023.

The group now operates over 50 communities in seven states, with a portfolio composed of 51 skilled nursing communities, 8 assisted living communities, 2 independent living communities, and 2 memory care centers. The range of services includes rehab therapy, respite care, and comprehensive post-acute support.

Its ownership structure is private and includes a significant merger: Evergreen Healthcare Group merged with EmpRes Healthcare in late 2023, combining resources and leadership to expand its geographic reach and care capabilities.

Founded only in April 2020, Evergreen is relatively new yet rapidly expanding. It is headquartered at 4601 NE 77th Avenue, Suite 300 in Vancouver, Washington.
