CommCare was incorporated on January 12, 1984, and obtained IRS 501(c)(3) nonprofit status on May 16, 1984. The company remained inactive ("dormant") until 1992, when it began acquiring skilled nursing facilities in Louisiana.

As a nonprofit corporation, CommCare operates through multiple wholly-owned subsidiary entities, including various non-profit real estate holding companies, a management company, an insurance provider, a pharmacy (55% ownership), and a hospice/home health company (55% ownership)—the latter extends operations into Mississippi.
