About Commcare Corporation

CommCare Corporation is a nonprofit organization (exempt under IRS code §501(c)(3)) based in Louisiana. It acquires and operates long-term care facilities such as skilled nursing, assisted living, and independent living communities. The organization also retains ownership stakes in related services such as a pharmacy and hospice/home health operations.

CommCare was incorporated on January 12, 1984, and obtained IRS 501(c)(3) nonprofit status on May 16, 1984. The company remained inactive ("dormant") until 1992, when it began acquiring skilled nursing facilities in Louisiana.

As a nonprofit corporation, CommCare operates through multiple wholly-owned subsidiary entities, including various non-profit real estate holding companies, a management company, an insurance provider, a pharmacy (55% ownership), and a hospice/home health company (55% ownership)—the latter extends operations into Mississippi.

CommCare operates in Louisiana and Mississippi, with at least 16 affiliated nursing facilities as of June 2026. It manages 2,488 extended-care beds licensed in Louisiana, plus 142 licensed beds managed for third parties; the organization also holds ownership in facilities via real estate and operating subsidiaries.

CommCare provides extended care through multiple modalities—skilled nursing, assisted living, independent living, pharmacy services, hospice, home health, and insurance services. Recent acquisitions via the Post Acute Holdings JV broadened its scope into home healthcare and expanded facility ownership.
