About Centers Health Care

Centers Health Care is a post‑acute care services network based in the Northeast that provides administrative, operational, and management support to an array of affiliated health care providers. It covers multiple lines of business, including skilled nursing, rehabilitation, home health, adult day care, assisted living programs, dialysis services, urgent care, laboratory services, and telemedicine.

Founded in 1996, CHC began with a single facility, Williamsbridge Manor in the Bronx. Since then, it has grown significantly by acquiring and turning around nursing homes at risk of closure, adding services over the years including home health care, adult day care, dialysis, assisted living, urgent care, and telemedicine.

Centers Health Care is privately held and was founded by Kenneth “Kenny” Rozenberg, who continues to serve as its Chief Executive Officer. The corporate structure is not described as publicly traded or REIT-affiliated.
