About Care Initiatives

Care Initiatives began in 1989 as a nonprofit organization and has grown to become the largest single operator of skilled nursing locations in Iowa. Since inception, it has reinvested earnings into its workforce, facilities, and community services, rebuilding and renovating multiple locations and enhancing specialty services such as private rooms and therapy amenities.

Today, Care Initiatives serves more than 2,500 residents and patients through 43 skilled nursing care locations, 2 assisted living locations, 7 independent living locations, and 8 hospice office locations. Notably, in 2024, the organization expanded beyond Iowa by launching a hospice office in Omaha, Nebraska.

As a nonprofit, the organization reinvests its earnings into staff education, infrastructure, and care programs. Since 2000, it has rebuilt 14 facilities and renovated or added to three more, while also contracting over 100 therapists for rehabilitation services.

Its service offerings include a continuum of care: skilled nursing, rehabilitation therapy, hospice care, assisted living, and independent living.
