In 2014, Beaver Valley Hospital began acquiring skilled‑nursing facility licenses statewide, and as a NSGO using the State’s Medicaid UPL program, grew from an initial 4 licensed skilled nursing facilities to owning or operating as many as 40–44 facilities.

Beaver Valley Hospital remains city‑owned, governed by Beaver City, with its hospital recognized as a governmental NSGO. It leases the real estate for nursing facilities at fair market value and contracts private management groups to operate day‑to‑day operations.

Its operations include providing “seed money” for Intergovernmental Transfers (IGTs) necessary to access UPL funding, with nursing facilities retaining 49% of those supplemental UPL payments after administrative fees and matching funds. Oversight includes daily communications, quarterly UPL reviews, and annual independent audits.

By mid‑2020s, Beaver Valley Hospital was responsible for caring for over half of Utah’s nursing‑home patients, operating dozens of skilled‑nursing facilities (reports cite between 40 and 44 in total).
